GeriatRx is a pharmacist-led clinical service focused on deprescribing, medication optimization, and patient-centered care for older adults. We collaborate with healthcare providers, care teams, and community partners to reduce unnecessary medications, prevent adverse drug events, and support aging-in-place with greater confidence.

Through our referral program, you can easily connect patients, clients, or residents to our services. Once a referral is submitted, our clinical team will reach out to the patient or caregiver within 72 hours to begin the intake process and coordinate care.
